NATIONAL ORTHOPAEDIC HOSPITAL DALA,
INVITATION TO TENDER/EXPRESSION OF INTEREST (EOI) FOR PROJECTS UNDER THE 2020 CAPITAL APPROPRIATION
In compliance with the provisions of the Public Procurement Act, 2007, the Hospital hereby invites tenders from Contractors/Consultants with proven track records to submit technical and financial bids /EOI for the design and supervision of the following projects:
2.0 PROJECT DESCRIPTION
LOT 1: CONSULTANCY SERVICE FOR THE DESIGN AND PROJECT SUPERVISION OF GENERAL OUT PATIENT DEPARTMENT
LOT 2: SUPPLY OF MEDICAL EQUIPMENT
3.0 ELIGIBILITY CRETERIA
Intending firms must submit all the under listed verifiable documents:
a. Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC2 and CAC7;
b. Evidence of Company’s income’s Tax Clearance Certificate for the last three (3) years valid till 31st December, 2020
c. Evidence of current Pension Compliance Certificate valid till 31st December, 2020;
d. Evidence of Current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020;
e. Evidence of Current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020
f. Evidence of Registration on the National Data Base of Federal Contractors, Consultants and Service Providers by submission of interim Registration Report (IRR) expiring on 31st December, 2020 or valid Certificate issued by BPP.
g. Sworn affidavit disclosing whether or not any officer of the relevant committees of the National Orthopaedic Hospital, Dala, Kano State or Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
h. Company’s audited accounts for the last three (3) years (2017, 2018, 2019);
i. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company’s profile with Curriculum Vitae of key staff to be deployed for the project, including copies of their Academic/Professional Qualifications;
k. Lot 1: Evidence of Firm’s Registration with the relevant professional regulatory body(ies);
l. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of awards, Valuation Certificate, Job Completion Certificates and Photographs of the projects;
m. Lot 2: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
n. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner);
o. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letterhead paper bearing amongst others, the registration number (RC) as issued by the Corporate Affairs Commission (CAC), and contact address, Telephone Number (Preferably GSM No.), and e-mail address. The Letterhead paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
4.0 COLLECTION OF TENDER DOCUMENTS (LOT 2)
Tender Documents are obtainable from the Procurement Unit, Administrative Block (1st Floor), National Orthopaedic Hospital Dala, Kano within the working hours of 8am-4pm (Monday – Friday) upon presentation of evidence of payment of non refundable Tender Fee of N10,000.00 only (for Lot 2) to National Orthopaedic Hospital Dala through TSA Account on REMITA Platform at any reputable Commercial Bank in Nigeria. Firms interested in Lot 1 will not pay at this stage until after shortlisting.
5.0 SUBMISSION OF TENDER DOCUMENTS (LOT 2)
Prospective bidders are to submit their bids in sealed envelopes the Technical and Financial bids must be prepared as separate document packaged in sealed envelope clearly marked as either “Financial or Technical Bid” as appropriate and inserted together in sealed single (bigger) envelope with the project title and Lot number written on the topmost part of the envelope and address to THE MEDICAL DIRECTOR, NATIONAL ORTHOPAEDIC HOSPITAL, DALA, KANO. Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop in the designated Tender Box at THE OFFICE OF MEDICAL DIRECTOR, 1st Floor, not later than 12:00 noon on Monday, 21st December, 2020.
6.0 SUBMISSION OF EOI DOCUMENTS (LOT 1)
Interested firms are to submit two (2) bound of Expression of Interest (EoI) documents separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope and addressed to THE MEDICAL DIRECTOR, NATIONAL ORTHOPAEDIC HOSPITAL, DALA, KANO and clearly marked with (the name of the project and the Lot number). Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop in the designated Tender Box THE OFFICE OF MEDICAL DIRECTOR, 1st Floor, not later than 12:00 noon on Monday, 23rd November, 2020.
7.0 OPENING OF EOI AND TECHNICAL BIDS
The EOI and technical bids will be opened immediately after the deadlines for submission at 12:00 noon on Monday, 23rd November, 2020 and 12:00 noon on Monday, 21st December, 2020 respectively in the Hospital Board Room, Second Floor, Administrative Block NATIONAL ORTHOPAEDIC HOSPITAL DALA, KANO. National Orthopaedic Hospital Dala will not be liable for misplaced or wrongly submitted bids. 8.0 GENERAL INFORMATION
a. EOI/Bid must be in English Language and signed by an authorised by the bidders;
b. EOI/Bids submitted after the deadline for the submission will be return un-opened;
c. All cost will borne by the bidders;
d. Presence of bidders/representatives and members of the public shall not be required during the bid opening ceremony in compliance with COVID-19 Guidelines. The Bids Opening will be covered by video recording with the presence of two (2) representatives of private sector professional bodies and Civil Society Organisation in the areas of Anti- Corruption/Transparency;
e. Lot 1: Only shortlisted firms will be contacted at a letter date for collection of Request for Proposal;
f. Lot 2: Only prequalified bidder at technical evaluation will be invited at a later date for financial bids while the financial bids of un-successful bidders will be returned unopened;
g. Civil Society Organization, Professional Organization and the General Public are invited to the openings;
h. The National Orthopaedic Hospital Dala is not bound to prequalify any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act, 2007.
